Enrollment Procedures

Proof of Residency: (parent or guardian) all documents listed below must be in the name of the person registering the student.

Category I (only need 1)

• Real Estate Tax Bill
• Mortgage Statement
• Agreement of Sale
• Signed & Dated Lease/ Rental Agreement

Category II (need 3 with current address)

Must have the name of the parent or guardian enrolling the student.

• IL Driver’s License/ State ID
• Auto and/or Voter’s Registration
• Gas, Water, Electric, Phone, Cable, Internet Bill
• Home, Apartment, Auto or Medical Insurance Bill/Statement
• Credit Card Bill/Statement
• Bank Statement
• Medical Card

Student Information:

The following documents are required except for those that are not applicable (identified with an *)

• Original Birth Certificate (not hospital certificate)
• Health Records
• Official Transcript/Test Scores from previous school
• If transferring from a school in Illinois we need the ISBE form * (provided by the school from which they withdrew)
• Special Ed Records (504, IEP) *
• Legal Custody Documents *

• Estado hipotecario (mortgage)
• Acuerdo de venta firmado y fechado
• Contrato de alquiler

Categoría II (Se necesitan 3 con la dirección actual)

• Licencia o / identificación del estado (o matricula)
• Registro de auto o votante
• Factura(cuenta) del Gas, Agua, Electricidad, Teléfono, Internet o Cable
• Seguro de Auto, Casa, Apartamento o Médico
• Cuenta de tarjeta de crédito
• Estado de banco
• Tarjeta médica

Información del Estudiante:

Todos los documentos se requieren, excepto aquellos que no son aplicables que se pueden identificar con un *.

• Acta de nacimiento original (no se acepta certificado del hospital )
• Registros (archivos) de salud (vacunas)
• Registros de calificaciones de la escuela / puntajes de exámenes
• Si se está transfiriendo de una escuela de Illinois necesitamos la forma ISBE (proporcionado por la escuela de cual el estudiante se salió) *
• Records Especiales ( 504 , IEP) *
• Documentos de la custodia legal *

Tome en cuenta: (El proceso de registración requiere todos los documentos mencionados para poder inscribir al estudiante.)
